Steps to Accept or Decline your FHEL Acceptance

  1. Confirm or decline your application by filling out this form. 
  2. If you plan to participate, click here to download the confidentiality form and sign. Once the form is signed, upload the form here.
    • Please name the form Last Name_FHEL.
  3. Click the "Register Now" button above and submit payment. 
    • 50% of payment must be received by July 1, 2024
    • 100% of payment must be received by October 1, 2024
